Do’s and Don’ts for Ear Irrigation
Previously known as ear syringing, which involved spraying water directly into the ear in order to loosen and flush out impacted wax, ‘water
irrigation
’ is the term for the more modern variant.
Water irrigation aims for a similar conclusion to water syringing but employs a much more controlled manner.

Normal saline solution is the most widely used irrigant because of availability, sterility, low cost and rare side effects. A previous study demonstrated that patients significantly preferred warmed solution for ocular irrigation, but no studies have demonstrated a similar preference with ear irrigation. Ernst and associates compared the comfort level and side effects of warmed versus room-temperature saline solution for ear irrigation. Forty volunteers, mostly emergency department person nel in a single hospital setting, underwent ear irrigation with warmed and room temperature saline solutions in a single-blind, randomized, crossover study.
Patients need to see a gp first and then if they are assessed as requiring irrigation they will be asked to use ear drops for 2 wks, then they can book in with the nurse or health care assistant for ear syringing. They must put the olive oil drops in twice a day for two weeks to make the wax loose enough in order for it to be irrigated. If a patient does not put it in appropriately it will not work.
A perforated eardrum, or if you have had a perforation in the last 12 months previously had problems with irrigation, such as pain in your ear or severe vertigo have a discharge of mucus from your ear or have had an ear infection in the preceding two months have recurring or persistent infections of the ear canal you have had any ear surgery (apart from cases of extruded grommets which had come out at least 18 months beforehand) had a middle ear infection (otitis media) in the past six weeks.

What Is Ear Syringing?
Harbourside family practice will no longer be able to provide routine ear syringing to our registered patient population except in specific circumstances. Why has this changed? surgery is contracted to provide ear syringing in certain circumstances, for example where requested to do so by secondary care (hospital services such as audiology or ear, nose and throat) or where a gp requests this because they need to make an assessment or a diagnosis. Surgery is not contracted to provide a routine service to patients who experience a build-up of wax. We have tried to accommodate patients as best as we can in the past but in reviewing our capacity and demand we have concluded that it is no longer possible to.

Ear wax arranegments from 1st janaury 2020 ear irrigating (ear syringing) is no longer recommended as first line treatment for blocked ears. Ear syringing can lead to ear infections, perforated ear drum and tinnitus (persistent noise). It is not a requirment to provide this service in general practice. This gp practice does not currently provide an ear irrigation service on request but is working with other gp practices in our primary care network (pcn) to look at this in the future. Most cases of ear wax can be successfully sorted using the self-help measures including, in line with nice (national institute for health & care excellence) recommendations, to use olive oil, which has been clinically proven to be safe and effective for the treatment of ear wax. Without the need for ear irrigation (syringing). A very small number of cases may require ear irrigation, and you will arrange a routine appointment for this.
Irrigation Ear Wax Removal
Some providers may choose to soften the wax before irrigation. Multiple agents may be used including mineral oil, 1% sodium docusate solutions, and carbamyl peroxide solutions. Warm the solutions and the water that will be used during the irrigation to near body temperature to prevent dizziness. Cold or hot solutions put in the ear are likely to have an uncomfortable effect on the patient, and it may make them dizzy or nauseous. If using an iv catheter and syringe, ensure the needle is removed from the iv catheter.
If ear drops or ear irrigation do not work, other options include: microsuction, which is a quick and comparatively painless treatment involving a small device which sucks out the wax from your ear. This is usually a safe option even if you have a damaged ear drum. An aural toilet uses a thin instrument with a hoop at one end to clean your ear and remove the wax.
